You appear to have an older version of migration-assistant in the CD you're using. Ubiquity 1.4.4 came with migration-assistant 0.4.2, but 0.4.3 added fixes for what I believe is the bug you're hitting. Could you run `sudo apt-get update && sudo apt-get install ubiquity` in a console and then run the installer again with the -d option. If it still fails, please attach those same log files again, if not, please let me know that everything went smoothly.
