I'm afraid your hard disk seems to be having problems. There's lots of
this stuff in your /var/log/syslog:

I'd make backups and consider replacing the disk. (Of course, it's also
possible that it's a kernel bug, for example DMA being turned on when it
shouldn't be or vice versa.)
