Unfortunately, I'm RMA'ing the drive which caused this. Before I removed it, it wasn't even being detected anymore, so I'm not sure even if I had the drive I could reproduce it. But, what I think happened is that the drive died completely (versus just not being reliable), and when it dropped off of the SATA controller the kernel locked.
