Okay, FWIW I must sheepishly report that I did find a workaround for
OpenMedSpel, at least. Changing my language (Tools-->Languages-->All
Text) from English, American to English, Canadian allows me to use the
Canadian dictionary and OpenMedSpel dictionary at the same time. So far
it appears to be working fine. Perhaps the American English dictionary
is somehow corrupt?

I'm running Linux Mint 8 on an eeepc 1005 with OpenOffice.org 3.1.1

I got the solution from Hagar of Ubuntu Forums, although editing
Dictionaries.xcu didn't work for me--I had to use the menus in
OpenOffice:
