I went back to karmic to run gtf and cvt - they produced slightly different modelines, but I was more concerned about them selecting 197.97 mHz as the pixel clock rate when the Dell specification and the EDID modeline says 156.75. I ran "cvt --reduced 2048 1152 60" and was astonished to see that it recommended EXACTLY the same modeline as Xorg reports when it decodes the EDID.
I have two different display adapter chipsets and three drivers that all generate desktop images that are too big or are misaligned with the physical screen. That eliminates a lot of potential causes. The problems occur on three different versions of Xorg, so the screen is heavily implicated. However, the EDID data matches the modeline recommended by cvt.
