With the default ext4 mount options, there is no bug :
rw,nosuid,nodev,relatime (well nosuid and nodev are not by default actually)

It is not the nobarrier option as it was not working with it either.
So it must come from the writeback mode (with optional nobh) and/or the 
commit=120 (which was used to commit data every 2min instead of every 5s).
