Thank you for reporting this issue. Was this while using a LiveCD? I ask
because I see many read errors in the logs. For example:

Nov 23 04:51:18 ubuntu kernel: [    8.093376] sr 1:0:0:0: [sr0] Add. Sense:
Unrecovered read error
Nov 23 04:51:18 ubuntu kernel: [    8.093383] end_request: I/O error, dev sr0,
sector 1360208
Nov 23 04:51:18 ubuntu kernel: [    8.093389] Buffer I/O error on device sr0,
logical block 340052
Nov 23 04:51:18 ubuntu kernel: [    8.442266] sr 1:0:0:0: [sr0] Unhandled sense
code
Nov 23 04:51:18 ubuntu kernel: [    8.442273] sr 1:0:0:0: [sr0] Result:
hostbyte=DID_OK driverbyte=DRIVER_SENSE
Nov 23 04:51:18 ubuntu kernel: [    8.442279] sr 1:0:0:0: [sr0] Sense Key :
Medium Error [current]

Which indicates an issue with the cd or the cd-rom drive.
