Could be a kernel bug. I had a problem with the middle button on my usb mouse. It was a kernel bug introduced between 2.6.20-rc4 and 2.6.20-rc5. My bug report is at http://bugzilla.kernel.org/show_bug.cgi?id=8099
A fix for a usb Logitech device killed the middle button for some usb mice.
