Me too have run into this problem, trying to create and format an sdb17 being the 16th volume (extended included) on the disk.
I think this problem has to do with the 15 volumes limit for scsi disks which also applies to sata disks. However, I use ide disks that should support at least 63 volumes each. I've seen even higher numbers, so this is definitelly a bug for ide disks.
