Synaptic 0.62.5 Kubuntu 9.10 "gtk-alternative-button-order" when set to 1 does not work. It should change the gnome HIG "Cancel OK" "Cancel Apply" button order to more kdeish "OK Cancel" Apply Cancel". It seems (for whatever the reason is) that gtk+ devs require some small extra code in the application to make this setting work.
