the pc that I was trying to get this wifi adapter to work is couple of
generation old - it did not have any wired or wireless adapters; I tried
this dynex pcmcia wifi adapter via a ricoh pci-pcmcia adapter that I had
handy; the results above. this approach was discarded and replaced with
a wired pci ethernet adapter that works with no problems.

I left this bug open with the hope that future kernels would recognize
the windows on the pci-pcmcia adapters and probe further those windows
for presence of pcmcia adapters and configure them.

personally I'm not affected by this bug any longer - as mentioned above
wired ethernet employed. currently there is no pci-pcmcia adapter
installed on this pc and I do not intend to install such in the future.
