Ah-ha! You have timidity running! It might be linked to Urban Terror or npviewer. Timidity has been known to cause issues with pulseaudio. As root, you will need to edit the /etc/default/timidity.cfg file. Find the line that says: TIM_ALSASEQPARAMS="-Os" and change it to TIM_ALSASEQPARAMS="-Oe" (so, change the 's' to an 'e'). Then restart timidity with the command: sudo /etc/init.d/timidity force-reload
This should, at the least, solve some of the issues.
