Ah-ha!  You have timidity running!  It might be linked to Urban Terror or 
npviewer.  Timidity has been known to cause issues with pulseaudio.  As root, 
you will need to edit the /etc/default/timidity.cfg file.  Find the line that 
says: TIM_ALSASEQPARAMS="-Os" and change it to TIM_ALSASEQPARAMS="-Oe"  (so, 
change the 's' to an 'e').  Then restart timidity with the command:
sudo /etc/init.d/timidity force-reload

This should, at the least, solve some of the issues.  There may still be
some other underlying issues causing the same problem, but this should
clear up one major contributor.
