Have this issue too, upgrading pulseaudio to 
0.9.21-0ubuntu2~~karmic~ubuntuaudiodev1 didn't solve the problem.
I have attached the syslog with messages from pulseadio & alsa.

After crash I see only 'clicks' instead of sound in speakers.
If I stop the music, run "pulseaudio -k" and press "play", the music plays
