I think the following line from your syslog:

Nov 10 22:55:28 main-menu[358]: (process:1330): FATAL: Module loop not
found.

means that your iso, though found, was never mounted. The kernel module
"loop" is required to do this mount.

Why it can't find this kernel module is a complete mystery to me.

How did you run "check disk for defects"? With this setup (hd-media
kernel and initrd plus alternate iso) I never see that option.
