There have been some changes and I would like to update this bug.
Changes now allow me to get the card running without re-booting the machine. 

Status:
When I boot up, my Creative X-fi card has about a 50% chance of being running. 
This is even though both of the card's lights are lit up, and asoundconf list 
shows the card properly. 
If it is running, and I open and then close a sound application, the card 
almost inevitably stops working. 

Change:
To get the card running again, I now can remove the card, re-insert it, and 
then modprobe for snd-hda-codec and snd-hda-codec-ca0110. 
I no longer have to re-boot. 
But I must do this almost every time I boot up, and every single time I stop 
and then re-start a sound application.

This is a very annoying bug because basically there are only two good
soundcards that use the expresscard slot, this one and the indigoiox,
and the indigoiox is not recognized at all under Ubuntu. (I have filed a
bug for that elsewhere.)
