This happened to me when upgrading ONLINE, not with a CD !

I upgraded from 9.04 to 9.10 via the update manager through the internet, I had 
no CD in the drive.
Yet I got the question "Please insert the disk .... Karmic Koala .. Release 
amd64 (20091027) .."

I just continued to click OK and the upgrade then continued "getting new 
packages".
It would get more files for 15secs or so, then it would pop-up the "insert" 
menu again, 
I would click OK, and so on, eventually it had all 1276 files.

Then it continued with the next phase of the installation, till the end, which 
was sucessful.
I did have a 9.10 ISO on my hard disk which I downloaded earlier,  but I did 
not use this ISO for the installation,
the installation was done via the net. 
This was done on an ASUS notebook
