Update on this case - I have changed computers this is in (now on an ECS GF8200A GeForce 8200 motherboard) with a NEC PCI add-in USB card. The dongle is attached to this add-in USB card (since I have another bug opened about the USB ports on this MCP78S chipset motherboard being intermittent).
