The mount command is failing:

Feb 15 12:08:20 ubuntu ubiquity:        In some cases useful info is found in 
syslog - try
Feb 15 12:08:20 ubuntu ubiquity:        dmesg | tail  or so

Sorry to make you run it yet again, but can you run it one more time and
when it crashes include the output of dmesg?  This can be obtained by
typing `dmesg > dmesglog` and attaching the dmesglog file to this bug.
