I've been using my mobile phone as a modem, using Siminn as a service
provider (manually adding the provider element to the
serviceproviders.xml file) and the only thing needed is the APN - the
name servers are apparently provided by DHCP:

<provider>
  <name>Síminn</name>
  <gsm>
    <apn>internet</apn>
  </gsm>
</provider>

If you like to include name servers as well, these are the ones that
were provided to me:

212.30.200.199
212.30.200.200
