I reinstalled Ubuntu and afterward the adapter would work at start-up,
then eventually fail:

$ dmesg | grep 2100

[ 8307.176463] ipw2100: eth1: Failed to power on the adapter.
[ 8307.176470] ipw2100: eth1: Failed to start the firmware.

After using the new install for two weeks, the adapter now fails on boot
the same way described previously.
