I don't know if this will help with solving this really annoying bug, but I've realised that Flash can suddenly stop working when I open another tab in Firefox that also contains Flash.
For example, just now I was listening to some music in a Flash player on one website, and when I opened a tab and navigated to another website that contained Flash, all Flash content just disappeared to be replaced by the grey boxes and the music playback ended.
