Having the document override the users preference for how the viewer should behave is very broken behavior, and in fact this is not how every Adobe's PDF viewer behaves (Adobe Reader 8 on windows). I just checked and If I set it to display the whole page at once like I prefer, clicking on the index items does not change that.
