I can confirm this issue with HP Compaq CQ45-203 AU. Its occurred after I have 
run command:
sudo alsa force-reload

I'm using that command and restart pulseaudio server to fix lose sound on 
firefox and some applications. Before that I have add the line:
 options snd-hda-intel model=dell-m4-1 enable_msi=1
to end of /etc/modprobe.d/alsa-base.conf because Ubuntu have not detect 
correctly my hardware:
00:14.2 Audio device: ATI Technologies Inc SBx00 Azalia (Intel HDA)

The output of command: `sudo alsa force-reload` is there:
