For X-Window (which is the software between card graphic driver and widget like 
GTK or Qt), the number of screens must not change. If screen is mirrored, you 
have only one physical screen. Else you have two independant screens or or big 
virtual screen mapped on two physical screens.
In the stone edge (aroung 1980), X-Window was conceived to use many screens, 
but programmers get used to think on a single screen.
So you need to reboot X-Window and all software which use it.

I think problem lies more in Gnome or KDE, which save session data.

Can you explain me what exactly you means by "the external screen is not
refreshed" ?
