First try attaching the monitor before you power on the machine, boot normally. If that doesn't behavor correctly please disconenct the monitor and connect it again, and then attach the xorg.log afterwards.
Finally, does the situation improve if you add: Option "AccelMethod" "UXA" into the "Device" section of your xorg.conf and then reboot?
