This has been implemented.  A new extension called 'extrude' is
available in Inkscape 0.45svn.  See attached screenshot.
Since it is implemented as an extension, it may also work with earlier
versions of Inkscape.  Copy the motion.inx and motion.py file from
Inkscape SVN into your /usr/share/inkscape/extensions/ folder, restart
Inkscape, and look in Effects->Generate from Path.  If your version of
Inkscape has a different menu structure than Inkscape 0.45, you may need
to edit the motion.inx file to suit.

Note that this extrude capability isn't perfect, as you can't re-edit
the extrusion on-canvas, and I've noticed a quirk or two, but hopefully
it should suit basic perspective needs.
