Any chance you can try a vanilla 2.6.28 kernel and see if you can
reproduce the problem there?   Other very interesting test points would
be 2.6.28-rc5, and 2.6.28-rc7.  Potential fixes that might have fixed
this are:

The first patch, which I suspect is more likely the fix, was merged into
2.6.28.8 and 2.6.28-rc6.   The second patch was merged into 2.6.28-rc8,
and isn't yet in a 2.6.28.y series yet, although it is in the for_stable
branch of the ext4 git tree.

Hence it would be interesting to see if the problem is present in
2.6.28-rc5, and fixed in 2.6.28-rc6.   (And thanks to whoever can do the
test, since I haven't been able to figure out how to replicate it on my
systems yet.)
