I've got also some trouble with PulseAudio, but because of cdemu. On
last upgrade of cdemu-daemon, I couldn't get any sound from my sound
card, and applications such as Rhythmbox or VLC could crash randonly
(but without sound anyway).

Actually, it was not related to PulseAudio, but to the configuration
file /etc/libao.conf, which was set to "driver=alsa". If an application
opens an alsa socket, there is a risk that no other application can play
any sound. /etc/libao.conf must contain "driver=pulse" to get
applications using libaio working with PulseAudio.

Check your /etc/libao.conf configuration, and check if any of your
package could use sound at startup (such as cdemu-daemon). Maybe this
can help you.
