This is a known issue with all nvidia drivers. From what I understand
the driver assigns unique refresh rates to different displays in order
to differentiate between them. In any case,  I've detailed the solution

Please be aware that by disabling dynamictwinview you might no longer get 
plug&play for external monitors. You can read NVIDIAs reasons for doing things 
this way here: 
http://us.download.nvidia.com/XFree86/Linux-x86/1.0-9755/README/appendix-g.html
(see "Dynamic TwinView" section).
