GLX is not the cause of this problem. I disabled it in my configuration weeks ago and still encounter this problem daily.
Based on reports here and in other bug trackers, and my own experience with this problem, I agree that it is most likely related to load in some way. Xinerama itself can be rather resource intensive, and the 2D performance of the Nvidia drivers is notoriously poor. This probably accounts for the fact that it is most often Nvidia users who report this problem, and that upgrading to a newer, better performing driver appears to 'fix' the problem. The upstream report at http://bugs.freedesktop.org/show_bug.cgi?id=18668 (previously linked by Thomas Jarosch) suggests a problem with animated cursors may be at the root of this.
