I was having this same issue. It was not tied to my extensions. It was tied to my profile. I went through removing individual profile files until I removed the formhistory.sqlite file and was able to clear the issue. My formhistory.sqlite file was 43KB when I removed it. I have saved the profile in it's broken state to a compressed file.
