You mean you don't have the top row for any of the windows? But in 4)
you say you are using the minimize button?

Hmm, maybe you have some strange settings in CompizConfigSettingsManager (ccsm):
Press Alt+F2, then type "ccsm" and go to "Window Decoration". You can play 
around with the settings there but important is the line "Decoration Windows": 
the entry should be "any". If not, then you will not have this top row on any 
window.

Another thing you could try: Alt+F2, type "gnome-appearance-properties",
then go to "Visual Effects" and click "Normal". This basically sets your
ccsm settings back to Default.
