Binary package hint: lyx
Way to reproduce:
insert a math environment(alt+m+m)
insert a matrix with \pmatrix
add a few rows with ctrl+enter(I added 2 so I got a 1x3 matrix)
type something in the topmost matrix item(I typed \cos t)
add a column to the matrix with alt+m+c+c and watch lyx crash
Version: Lyx 1.5.6, Ubuntu 8.10
