This has also happened to one of my machines running Ubuntu Hardy. I have the backports repo enabled.
It occured when upgrading from linux-ubuntu-modules-2.6.24-19 => linux- ubuntu-modules-2.6.24-21 . My network log shows the same activity as yours did, i.e., seems to associate fine, and makes several DHCPREQUEST's before eventually giving up. This ONLY occurs when attempting to connect to a WEP enabled connection (haven't tried WPA), and happens every time. If I disable security, the connection performs fine however...
