Luke, I had the the latest 0.9.10-2ubuntu6 along with other updates on
the system and that is when I found this issue.  So after downgrading
alsa-utils and alsa-plugins, which didn't work I downgraded pulseadudio
and found that my setup worked again.

The thing I have not tried is to see which build of pulseaudio broke
things since i went strait to 0.9.10-2ubuntu2 from 0.9.10-2ubuntu6.  If
i had to guess, I think it is the 0.9.10-2ubuntu4 build that changed
something.
