Public bug reported:

Currently mkinitramfs simply overwrites the existing initrd.img, without making
a backup.
Yesterday I upgraded initramfs-tools, which caused it to quietly create a new
and broken
initrd.img (why it was broken is another story). I discovered this on my next
boot, which
failed unpleasantly. [It was at first a surprise since, after all, I hadn't
upgraded the kernel,
so why should it fail all of a sudden?] But unfortunately I wasn't able to
specify the old
initrd.img in grub, because it had been deleted, replaced with the new broken
one. Sitting
there with a non booting system, it became very very clear to me that keeping a
backup
copy of the old initrd.img would be a very good thing to do!

** Affects: initramfs-tools (Ubuntu)
     Importance: Untriaged
         Status: Unconfirmed
