After about an hour, the upgrade completed and system rebooted only to not ask for passphrase and system drops to busybox shell since the / LVM volume wasn't found. Fortunately, I still had the older 3.5.0-27 to choose from grub menu and it prompts for passphrase and I can boot the system this way but attempting to boot the 3.8.0-19 kernel never prompts for passphrase.
