Title:
  Noise in left channel with USB audio device

Bug description:
  Binary package hint: pulseaudio

  I keep hearing (white?) noise in the left channel of my headphones
  through a USB audio device.

  I just noticed it start in the middle of playback.

  I killed the /usr/bin/pulseaudio process, waited for it to re-start,
  and that made the noise go away.

  Power cycling the device repeatedly does not make the noise stop.

  The device is, from lsusb:
