This can happen when I play music with Spotify, talk with Skype or play with Steam. The only way to get the sound back is to restart the computer. I tried to follow the sound troubleshooting procedure (https://help.ubuntu.com/community/SoundTroubleshootingProcedure) but it didn't solved my issue. Attached are the information generated by the also-info script and the pulseaudio log taken while the issue occurred.
