The footsoldier, the house surgeon was the one who took the brunt of most of the attacks; theconsultants safely cocooned in their rooms. In a Govt. Medical collegeEmergency ward, the half a dozen house surgeons with hardly any experience wasno match for the huge rush of patients arriving with variety of seriousillness, many hailing from low educational background and a high explosivemindset. So the very first lessons that we were taught by our seniors, after wegraduated, was not algorithms or guidelines of diagnosis or management of a disease but ways to keep us safe from the irate crowd. Lesson 1. “Don’twaste time on examining patient for a very long time, people will think that you areinexperienced or incompetent. Examine quickly, Start a IV fluid, add a vial ofB-Complex injection in the fluid; till the seniors come in”. The IV saline would make the treatment look seriousto the bystanders, the harmless B-Complex injection rendering a deep yellowcolour to the saline to add to the drama. Lesson 2. Nextthing was to loudly announce “we mightneed blood transfusion”. Turn to few of the most vocal ‘tough-looking’bystanders and announce “some of you please wait, you might need to donateblood”. This ensured that in the next 10 minutes mostof the ‘tough-guys’ would gently disappear from the scene. Lesson 3. Towhomever was left, we were told to declare that the situation is serious and we are tryingour best. And garnish with a phrase like “might have been different if thepatient was brought in little while earlier”. Theses teachings did not make us great doctors, but did save us from gettingbeaten-up; but like every other teaching once in while they did gowrong. One of our friends saw an unconscious patient broughtto the ward in a trolley; in a dirty, torn cloths with a neglected look; aftera cursory examination he announced that “things might have been better if the patient was broughtat least couple of hours earlier”. “But sir two hours earlier he was eating lunch with us, and we had noidea that he would go and climb the coconut tree in the backyard from where hefell down just now”.
