Why can't you use procmail and just send the mail marked with **SPAM** to a different folder like "SPAM"? This is the typical setup used by SpamAssassin. That way you don't need to modify SM at all.
In your .procmailrc you would put a line something like this:
:0:
* ^Subject: .*\*\*SPAM\*\*
/home/yourusername/Spam
