There is small plugin called bug_report. It is included in squirrelmail
package. Install it. Go to Options->Display prefs and enable bug report
button (Bug Reports: [ ] Show button in toolbar)

Use "Bug" link that shows up in menu bar. Start bug submision. Then you
will see information that would ease debuging of your problem.

Developers need information about your config in order to understand your
problem. When you call your doctor, you don't say that you are sick. You
tell him/her that your [insert your body part here] hurts and doctor asks
to show your tongue or SSID. :)

It is possible that some plugin breaks things. Disable all of them and try
to reproduce your problem. Enable them one by one and try to reproduce
your problem. Set php error_reporting to E_ALL and check your webserver's
logs.
