That problem combined with the interesting behavior we saw trying to get
vlogin to work for you says to me that there is something unique about
your setup.  I am not sure where the problem might be, but it might be
instructive to review the versions, compile options, and configuration
settings for Apache, PHP, and SM.  You are in linux, no?
