We found the problem.
We had cookie persistence turned on the load balancer but we were
terminating the certificate on the server.
Apparently to have cookie persistence you have to terminate on the load
balancer.

That was a typo. We are using *:443
This is what I perceive to be the significant part of the error_log file
with LogLevel debug. No entries in either of the ssl log files ...
