Thank you for mentioning that you can manually add the device. My mouse
wasn't paired even using the hardware buttons. Going to the bluetooth
manager and selecting the mouse did work. I've added the udev rules in
the hope I won't need to do this manually each time. Thank you.

Instead of trying the 3.5-rc2 kernel listed above, the first time, I
tried the current mainline 3.6.0. Unfortunately this bug is back in
3.6.0.
Using 3.5-rc2 does fix this bug.

Utilizing the amd64 alternative install CD for 12.04, 12.04.1, 12.10
results in failure during "Select and Install Software" step.
