Ok I have found the problem - wrong boot loader device selection. I have
left the preselected /dev/sda which did not work together with /dev/dm-1
raid selection.

I have run a debugging session from the Eclipse side. When you click the
global menu it is producing event that gets handed down to the main SWT
loop, there Eclipse takes the event and gives it back to GTK calling
gtk_main_do_event for precise targeting of the affected component.

I can confirm reappearence of this problem in 12.04 both 32-bit and
64-bit, both clean install and upgrade. For me it is about accessing
WebDAV rather then SMB. It is working flawlessly in 11.10 but in 12.04
you can only get a sound for a second and then silence. Both Rhythmbox

Window is no longer grabable after de-maximize usin
While the crash was pretty deterministic, there was a simple workaround
for it. Install with NY timezone instead of the default one (CZ GMT
+1). It pick the main mirror instead of the local one and everything
went smooth.
