You need to be careful with manual partitioner. Make sure to have a
separated /boot (even on raid1 is fine) and another raid for /. /boot
needs to be at the beginning of the disk and possibly not bigger than
4GB (usually 512MB are ok).
You want to note 2 things. The very first partition cannot be
kernel fixes this behavior.
I need to know your setup basically and see if i can reproduce it here.

then you did dist-upgrade without configuring cman (redhat-cluster-suite) or

Here is the proposed patch for dapper.
Problems:
- init script does not perform proper check before attempting to
initialize hsg80 multibus failover
- hsg80 failover is not handled properly causing multipathd to spin 100%
of the CPU without readding the proper device to dm-multipath
Solutions:
i did try to check for the vendor but it's tricky because it depends
from the HSG firmware release.
For instance on mine it's still DEC (old firmware) but i know that most recent

This is probably network manager that is resetting the interface. Can
you try to stop the booting before entering runlevel 2, either purge
network-manager or edit the files in /etc/network/* related to network

After you did change the harddisk, did you try both with and without
PATA/SATA?
so the first problem was an hardware failure, right? if you get up to
gdm, the kernel isn't at fault here.
