As a work-around, I've noticed on the B+ that if you keep the Ethernet
disconnected until it booting and then connect it when you see it hit
"... Networking" then it'll pick up DHCP and then run cloud-init next
happily.

Cavium driver bug that I believe they're working on.

I do not know what happened to /etc/init.d/ondemand - it definitely
doesn't exist on that system and I did not manually remove it. It
doesn't appear that I have disk problems so is there any other way that
file could have disappeared? Where do we go from here?

So the question is why not attempt to install the uInitrd and boot
whether or not it's present? It's a rare case for Ubuntu to not provide
an uInitrd; if we have pre-programmed uImage and uInitrd memory
addresses why wouldn't we try and load both (not just uImage) and then
boot?

Hope this helps,
I was able to boot Server 2008 R2 and run with Maverick's seabios
bios.bin (0.6.0-0ubuntu1) file on Lucid on a 16 cpu (2/4/2) system
without a problem. I could not boot more than 4 cpus before with
Lucid's seabios (0.5.1-0ubuntu2).
Working configuration:
Linux ubuntu01 2.6.32-25
Clarification:
"... I could not boot more than 4 cpus ..." meant that I hit the BSOD
with Stop A5 (ACPI) when I tried, so it did boot but just didn't get as
far as I hoped.
