It happens not very often with me, usually when I reboot Access Point and
Ubuntu just don't want to find AP again. When I turn off and turn on wifi - no
networks at all.
Whank you for temp solution: "sudo iwlist scan" works much better then reboot =)

I have the same bug: a headset doesn't work in HSP/HFP mode
The firmware(BCM20702A1_001.002.014.1315.1410.hex) is successfully converted
via hex2hcd and placed to /lib/firmware/fw-413c_8197.hcd
any hints?
I see only these errors in the syslogs:
