Two fresh installs of Jaunty and this bug persists also I'm not sure if
this is because of this bug, but I thought it might be worth mentioning,
that I get no preview images in the gnome-appearance-properties when
browsing icons some do appear for controls but not all. screenshots:

Using 9.04 it's a fresh install I still receive the system freeze upon
inserting a cd.

As a follow up this error still occurs, most times without the error
message and only results in a system freeze, which leads to a hard
reboot.

I'm on Kubuntu Karmic and I get this error

Adding /usr/lib/kde4/cervisiapart.so works for stopping the error
message but as of Karmic this is still does not fix the problem. Any
news on a fix?

I would love to be able to tell you whether this was still an issue with
the latest version of Ubuntu, however I no longer have that system.
However I can tell you the mic did not work on any versions of ubuntu I
used on it the last being Oneiric Ocelot 11.10.
